Required Forms

The forms listed below are mandatory for all 7th and 8th grade students. 



Emergency Care Form Form required for all students. Completed via weCare.
Health Form

Only for students with medical concerns. Email completed Health Information Form to Public Health Nurse Pamela Manuyag (@email)

Computer Opt Out Form Only for students who are not taking a school computer.
Software Parental Consent Form 

Review software used at TMS and complete consent form.

Student Rights and Responsibilities (SR&R) Parent Signature Sheet (coming soon)

For information and to view the SR&R document visit the FCPS SR&R page. This form will be collected during our Eagle Time classes in the first month of school.

After School Registration Form (coming soon) Complete this form if your student is participating in the After School Program for the 2022-2023 school year.

New Student Orientation - Thursday, August 18 from 9:00 AM to 11:30 AM 

  • For students only
  • For all rising 7th grade students and 8th grade students new to Thoreau
  • Student will be able to follow their class schedule and meet their new teachers
  • Bus transportation provided to and from Thoreau from feeder elementary schools

Back to School Open House - Thursday, August 18 from 1:30 PM to 4:30 PM

  • 1:30 PM to 3:00 PM Suggested Last Names A-K
  • 3:00 PM - 4:30 PM Suggested Last Names L - Z
  • All Thoreau Middle School students and families
  •  Pick up a copy of your class schedule
  •  Learn about after school programs and activities
  • Drop off medical forms and medications
  • Pick up pre-ordered school supply kits from our PTA
  • Use MySchoolBucks to pay class fees, purchase PE uniforms,  or add money to your child’s lunch account
  •  Walk the building to locate your classes and meet your teachers.

  • Students class schedules will be available in StudentVue and ParentVue on August 18th at 6am!

PE Uniforms

  • All students are expected to “dress out” for Health & Physical Education (HPE) class.  We encourage students to change into the Thoreau HPE uniform, which can be purchased on or at our Open House on August 18th.  Students will not “dress out” the first week of school as teachers will be reviewing expectations and answering questions directly with students that week.
  • Each student will be assigned a locker in the HPE locker rooms at the beginning of the year. Students may store their HPE supplies (uniform, sneakers, deodorant) in the locker for the year while taking it home periodically for washing.  In addition to active clothing, all students are expected to wear proper tennis shoes for safe movement during activity.  Tennis shoes or sneakers are made to support and to help ensure your feet and ankles are sturdy and protected with good traction. Please note that Crocs, UGGs, and sandals are examples of inappropriate HPE shoes.
  • It is important to us that all students have access to appropriate clothing and sneakers for class. If any student or family needs support in purchasing these items please talk with your assigned HPE teacher or counselor and we will ensure your child has what they need to participate fully in class.

Bus information

  • Transportation assignments for the first day of school be available on August 09, 2022, in SIS ParentVue account on the “Student Info” tab.  The office of transportation may make changes to the bus schedule up to August 17, 2022.

Laptop Distribution

  • Thoreau students will receive their laptop on either the first or second day of school in their PE class.


  • Lockers will be granted by request. Students will be allowed to carry a bookbag from class to class. After the second week of school, students will learn how to formally request a locker after having the opportunity to experience not having one.
  • Students will be assigned a PE locker and these are not by request

FCPS Student Information System (SIS) ParentVue

  • SIS ParentVUE provides parents with access to student information, including attendance, report cards, class schedules, gradebook, course history, discipline, health, and school information.

  • This site allows Thoreau to collect various student fees such as PE uniforms, curricular materials fees, field trip fees, etc. 

7th Grade Immunization Requirement Notice

All students entering 7th grade will be required to receive one dose of each of the following immunizations prior to the beginning of school: Tetanus, Diphtheria and Pertussis booster (Tdap), and Meningococcal vaccine (MenACWY).          

7th grade students cannot begin school in August without proper Tdap and Meningococcal documentation.

Please provide one of the following documents to Thoreau MS  by Aug. 15th. You may drop immunization records off in the main office or email them to Karla Macoto at @email 

  • An updated immunization record showing that the student has received these immunizations.
  • A note on a healthcare provider’s letterhead stating the date the vaccines were given.
  • A healthcare provider’s written statement of a permanent or temporary medical exemption.
  • A notarized Religious Exemption Form 

Immunizations are available from your student's healthcare provider or the Fairfax County Health Department. The Health Department clinic locations and hours are available online or by calling 703-246-2411.
